Parkinson's, Nurse Specialist, Nurse Service

The Parkinson’s Nurse Specialist works closely with consultant neurologists and geriatricians to provide on-going care and monitoring for people with a diagnosis of Parkinson’s or a Parkinsonism.

They are part of the wider community nursing service in Guildford and Waverley working with patients, carers, GPs, therapists and other specialist nurses.

The service aims to optimise health and wellbeing for individuals through education, specialist advice, support and treatment to optimise health and wellbeing at home and to prevent unnecessary hospital admissions.

Who is the Parkinson’s Nurse Specialist Service available to?

The service is available for adults aged over 18 who are registered with a Guildford & Waverley GP.

This includes patients living in their own homes or residential homes and nursing care homes.

How can I arrange an appointment?

Patients can self-refer or be referred by a GP, hospital consultant or another health & social care professional.

If you are already known to the specialist nurse and you have a question or need to change your appointment, please contact us by ringing the Community Coordination Centre on 01483 362 020.

If you require an interpreter to be present at an appointment, please contact us as soon as possible so that we can arrange this.

Where are appointments held?

  • Appointments will be in a clinic setting or at home if the patient is housebound.
  • Clinics are held at Milford, Cranleigh and Haslemere Hospitals.

  • Telephone consultations are also available.

What will happen at an appointment?

  • Assessment and care planning to develop and agree individual goals.
  • Education and advice to support people with treatment and self-management strategies at home.

  • Optimisation of and medication review and advice.

  • On-going support, management, monitoring for any deterioration in condition.

  • Referral where needed to appropriate services.

What should I bring to my appointment?

Please bring details of any medications that you are taking.

When is the service available?

The service operates Monday to Friday 09:00 to 17:00, excluding bank holidays.

However if you would like to speak to a generalist nurse at the weekends or out of hours, please call our Community Co-ordination Centre on 01483 362 020.
