The Hospital Outreach Support Team is made up of Occupational Therapists and Physiotherapists who aim to make the transition from hospital to home as simple and as safe as possible. We do this by providing short term therapy, adaptive equipment and organising additional services in your area if deemed necessary.
Referrals are taken from all wards throughout the hospital including A&E, or sometimes directly from a GP or Social Services to prevent admission or re-admission. We see our patients at their own home, usually the day after they are discharged, however sometimes we will meet patients on the ward prior to their discharge. We cover a 20mile radius from the hospital.
You will be referred to HOST by your ward therapist if there are short term goals you wish to achieve, or to address potential problems you may encounter when you return home. These may include: improving safety and confidence with your mobility, increasing independence with activities of daily living, identifying environmental hazards or identifying adaptive equipment which may be beneficial.
We do not take self-referrals but you can talk to the therapists on the ward if you have concerns about returning home and they can either refer you to our team, or to a more appropriate local service if indicated.
Your length of treatment and number of visits will be determined by the therapists who see you and can range from a one off phone call or visit to a maximum of four weeks of treatment. If a longer period of treatment is required our team will assist in referring you onwards to the appropriate community services.
