Learning Disability Liaison nurses provide advice and support for all people with learning disabilities, family carers, paid staff and acute hospital staff.

You can register that you have a learning disability and the help you will need by completing the form below and returning to the hospital you attend the most.
We recommend that you complete a Hospital Passport ‘This is Me’ to help staff within the hospital understand your needs. This document belongs to you and can be used in all Surrey hospitals as a inpatient and outpatient.
