Adult Community Health Services

The Adult Community Health Services Team provide health care services to adults across Guildford and Waverley. We support and care for patients in the community, maintaining their health, independence and preventing unnecessary hospital admission.

Our service comprises community nursing, podiatry and therapies teams. They also offer care at home for patients as well as within our community hospitals.
