Home First

The Home First team visit people in their own homes once they are medically fit to leave hospital but still require short term care and rehabilitation.

If people require longer term care and support the team may refer on to social services.

Home first provides up to 72 hours support and home based assessment of your needs and a therapy plan if required.

It is a joint initiative between the Royal Surrey, Adult Social Care (social services) and Guildford Borough Council. 

How can I arrange an appointment?

This would be by way of referral (with your consent), from the hospital following your discharge.

Where do I go for the appointment?

The Home First team will visit you at home on the day of discharge or the next day. This will be dependent on your needs.

The hospital team will discuss your requirements with you before discharge.

What can I expect at the visit?

The team will discuss your needs with you based on the recommendations from the hospital team.

A therapist from the team will complete an assessment with you which will include goals and a plan of care. This may include a carer assessment as required.

Your visit may include all or some of the following:

  • Support/goal setting with personal care (morning and/or bedtime)

  • Support/ goal setting with meal preparation

  • Support with medication

  • Review/ goal setting of your mobility and transfers 

  • Review of you home environment and risks review/ goal setting of equipment required to support your rehabilitation and ensure your safety

 

What happens next?

During the first 72 hours the Home First team will review your needs with you in terms of any requirement for further care, nursing or therapy.

This may include referral to other services for ongoing needs.

What will I need for the visit?

Please share your personal details, giving your next of kin contact details and details of medication that you are on or any hospital discharge letters.

The therapist or Home First team member may ask to have access to different parts of your home as part of their environmental assessment.

When is the service available?

We work 365 days a year between 7.30am - 21.00pm.

We very much value feedback from all of our patients, and may ask you to complete a satisfaction questionnaire after your appointment.
