The Community Co-ordination Centre (CCC) is a single referral point for health and social care professionals to refer local residents registered with a GP in Guildford and Waverley. Residents are also able to self-refer.
Once we have received the referral, we will assess each individual and co-ordinate the appropriate community services to support the patient’s health and wellbeing to enable to patient to remain in their own home and to prevent any unnecessary admissions to hospital.
The service is available for adults aged over 18 who are registered with a Guildford and Waverley GP.

This would be through a referral from your GP, health or social care professional or following a discharge from hospital.
You are able to refer yourself by calling the Community Co-ordination Centre on 01483 362 020.
When your referral is received either by email or phone an administrator will ensure we have taken a detailed history of what your health concern is to enable the clinician to identify the appropriate services to support you.
The services identified will then contact you to arrange an appointment.
The crisis response team will visit you at home this may include a nurse and therapist to assess your needs in a timely manner.
Please share your personal details, giving your next of kin contact details and details of medication that you are on or any hospital discharge letters.
Please provide the visiting community service with hand washing facilities and paper towels if you are being seen at home.
We work 365 days a year working from 8am – 8pm each day.
